I just replaced a failing drive and am posting notes & observations for others to benefit...
4 years ago, I built a NAS as-shown below and it's been working great. The system is checked regularly. It is on UPS and usually runs continuously for periods of 3-5 months in-between physical maintenance to clean the fans etc. The system has two Z pools. Each pool has 2 mirrored drives; one pool stores files from 8 streaming surveillance cameras, the other pool is local home backup storage for family PCs. The video camera pool is in constant use!
One of the drives in the video camera pool started to show varying reports about Raw Read Error such as:
SMART Prefailure Attribute: 1 Raw_Read_Error_Rate changed from 118 to 116. No other drives were reporting this so I decided to replace the drive. It had 35078 Power-on hours (4 years, 1.5 days).
I followed the instructions from this YouTube video which worked with one minor glitch.

When doing the "replace" operation, if anyone gets an error message like: "cannot open 'ada0p1': no such GEOM provider", the new drive needs to be formatted the same as the old drive. In my case, it was necessary to format the new drive with "Create ZFS on GPT partition" because that's how I setup the original drive. Formatting was done through the GUI and afterward, the "replace" operation was successful and it started to resilver.
Once resilvering was complete, for some reason, the SMART logs were not being produced. By going to Disk->Management and re-enabling SMART monitoring for each drive, the logging resumed. Everything is back to normal now.
Minor glitch while doing the swap but fortunately only took 30 minutes to take an educated guess about corrective action. Happy ending...
