CAM status: CCB request completed with an error

1 USB Disk is always missing at start up. I noticed during boot it hangs for a minute with:

However, if I unplug and re-plug in the disk it works fine. It's a brand new 2 port powered PCI Express USB 3.0 board and the other disk plugged into it works fine.

What does the above error mean? It also appears in the system log after I click "Rescan Disks".
