On console, there is always a UPS event (even though I do not see the UPS drop
or any issue with site power):

unplugging/replugging drive does not help. Reboot does. Have replaced drive with
spare to no avail, as well as replacing cables. Was happening on a 9.1.0.0 revision as well

That only leaves the cable or the port. I would start with swapping the cable for a known good one with locking tabs.

sometimes is the box case ( in the case that your disk is in a extraible drive bay).
12.1.0.4 - Ingva (revision 7743) on SUPERMICRO X8SIL-F 8GB of ECC RAM, 11x3TB disk in 1 vdev = Vpool = 32TB Raw size , so 29TB usable size (I Have other NAS as Backup)

I had already tried swapping the cable for a known working one from another system (and swapped the one from the NAS into the other). There was no change in either system.
I don't use external bays or hot swap trays or backplanes of any sort.
OK, I wanted to just verify it wasn't something like a software issue with my specific raid controller card or something of the sort.
Any clue why this would be coupled with a loss of comms to the UPS though? That to me signified that it may be something other than just a bad port/cable/drive.

Sounds like ports going bad on the motherboard. I would get a new one quickly.
