Dying Controller, or firmware/driver issue?
I have been using NAS4Free for about 2 years on the following build:
Dell SC524-SC
-Dual Xeon L5420 2.5Ghz Quad-Core processors
-16GB RAM
-IBM M1015, flashed to SAS2008 using P15 firmware, IT mode
-Seagate Barracuda 3TB x 4 - connected to the M1015
-WD Green 1.5TB x 2 - connected to server backplane
-WD Green 2TB x 2 - connected to server backplane
NAS4Free 9.3.0.2.1349, embedded
I started seeing intermittent issues around mid to late last year when I upgraded to release 9.1. The issue would clear after a reboot. The reboot process however had to be manual. The system would start shutting down and hang up during the shutdown process, thus never completely restarting. I would have to power cycle the server to complete a reboot. I thought I must have a corrupt file somewhere in the OS.
At one point I upgraded the release to 9.2 to see if that cleared the issue. I didn't see it for a while, then it came back. I blew out the entire OS (format and reinstall). 9.3 was out by then, so I went ahead and installed the newest 9.3.
Sure enough, the issue came back. I get repeated controller mpt_cam_events (0x80), then eventually have to power cycle the machine. This seems to happen more often when I put the controller under heavy load.

I am not sure if I have a dying controller here, the FreeBSD driver isn't working correctly, or I need to update the firmware (driver<->firmware conflict). Any help would be greatly appreciated. This machine and my NAS4Free build are used in my home heavily, streaming multimedia around my home each day.

Just thought of something while looking at my config file..
My 1015 (SAS2008) is an mps controller, while the backplane (or onboard controller) is mpt. With the errors being mpt0, would this be an issue with the onboard controller, or a driver issue with the onboard controller? The onboard controller I believe is a SAS1068e.

On actual Nas4free its P16 driver, so flash P16 firmware, this can fix this.

I'm seeing this problem too with a Dell SAS 6/IR controller. I'm looking to update the firmware for this pciex8 card. Dell seem to have one from 2013 but it is the same version number as the 2008 one already installed. Any ideas anyone?

maybe u can crossflash to lsi firmware.(u dont need the newest Firmware u need Codeset P16, newest is P20 from LSI)
What is the exact name of the controller, what chip he uses?.
Iam running an intel sas module, an Hp221 annd ibm 1015 all with LSI it firmware.
I could resolve those Problems with matching Firmware and driver number.
For 9.3 Nas4free u need Codeset P16 Firmware.

After looking at the BIOS when I got home the other day, I noticed the controller is actually a 1064E. It looks like the firmware release for the 1068 however, covers the 1064 as well.
Back when I flashed my M1015, I remember finding a page that instructed you how to make the boot stick in order to start the flashing process. I cannot for the life of me find that old thread.
So far it is up and running ok, but it tends to fall over during the weekend, when everyone is home and streaming media from the NAS.

Run fromm ssh
sysctl -a | grep mpt | grep version
output looks like this(this is from my nas mps i for LSi 2008 and newer)
look for Firmware and driver version they should match, if not, flash the Firmware Version witch matches the driver.
U can look if dell has the right Codeset Firmware or u can try Crossflash this thing.
If you need help with crossflash let me know.

on my Dell SAS 6/IR with the firmware upgraded to version 21 using the links above does not yield any output.
I am still seeing multiple errors reported on da0:mpt:0:0:0:0. This is whilst rsync is running and transferring files at 60 MB/sec.

if nothing comes out use
sysctl -a | grep dev
and post the output.
21 is the newest put possible to new.
With mps driver, all FW over 18 are giving the same errors with driver 16.
